How to set up a binder for your special needs child .

Thank you for visiting! I wanted to help you by showing you how I have set up my special needs son’s binder! This binder is packed full with all of his information from diagnosis to school and more! Let’s get started!

I have kept a binder for my son since he was a tiny baby, several, actually. He has had appointment after appointment, SO MUCH paperwork, and I have kept EVERY SINGLE PEICE! I have his binder in several sections. I will go over each section with you with examples of the information in them. I will try to be as detailed as I can!

But first, get yourself a thick and heavy duty three ring binder. Like the one here.

Section one.

In section one is where I have his basic information. This includes:

Name: Conner

Date of Birth: 1/1/01

Birth weight: 4.6

Health issues: Failure to thrive .

Pregnancy issues : None

Immunization records: Place a copy

Medication list: Tylenol as needed , Melatonin 1 tablet as needed at bedtime.

** I also keep his medication list on my phone, you’re gonna want that list with you. It’s hard to remember on the spot! Especially if they have a long list.**

(This is not his real information other than his name. This was for the purpose of giving examples.)

Section two.

This is where I keep all contacts. That would include doctors, agencies that we have used for medical supplies, feeding supplies, in-home nursing etc. Examples: (Not real names, numbers etc)

You get the idea! I have every doctor listed that he has ever seen, case workers and the company they are with, nursing agencies, insurance companies and so on. This will come in handy if anyone has any questions about other doctor’s reports, or if you need to get in touch with someone for whatever reason. That will happen a lot!

Section three.

This is where I keep appointment letters, appointment notes, my own notes, hospital stays and E.R. visits and such. This section is in sub sections. Here is the set up.

Primary Care

Genealogy

Urology

Endocrinology

Neurology

So, If I have an appointment report for Urology, it goes under Urology. If he has had a hospital stay, it would go under Hospital stays and so on.

There is also a zipper pouch where I put DVDs of x-rays, images, or any other kind of disc.

This section is pretty full. I have kept everything from every appointment that he has ever had.

Section four.

This is the therapy section. I keep all of his notes from Physical, Speech and Occupational therapies here. I want to be able to look back at his achievements. Also, to have proof of goals, and agreements. If something is done that you didn’t agree on, you will have your agreements as proof! Do not skip this! You may end up needing these papers one day. I could tell you some stories, but that is not the purpose of this post.

I also keep my own notes here so I know what he did or didn’t like, what he had a hard time with, etc. I keep logs of activities and printouts of exercises that we are given as well.

01/03/2004 Physical Therapy

‘Today Conner worked hard and did pretty much everything they wanted him to do. He got a bit irritated when he had to get up and walk but he did it! He walked for 10 minutes before he stopped to sit down and decided he wasn’t participating anymore today. Mr. Josh recommended that we use the stairs at home and have him practice going up and down a couple times a day. I’m happy with his session today! 🙂 ‘

Section five.

Results! I keep all results from labs and tests here. I like to keep them in their own place so they are easy to locate when I need them. I have them in order from newest to oldest. That simple!

Section six.

Resources!

I keep all resources, including ones I’ve used and ones that I may use in the future. This includes, The Ronald McDoald House, The M.O.R.G.A.N. Project, nursing agencies etc.

Section seven.

And finally, my last section is for school! Here I keep IEP’s, reports, school work etc.

I have homeschooled all but two years of Head Start and half a year in Kindergarten. We had some issues with some of the teachers so I decided to withdraw him for public school and homeschool him instead. (We won’t get into all that in this post though.)

I have a separate binder for homeschool, one per year actually. I just keep the basics here in section seven. I will write a separate post soon about setting up a homeschool binder, so keep an eye out!

Of course you will want dividers and tabs for your binder. Also, always make sure that you keep a notebook/paper in the back of the binder so you can write down any notes or numbers that you want to add later.

You can change things up to suit your needs. But with all this being said and using this as your guide, you should be well set! Let me add to that I use page protectors for everything as well! You can get everything you need at the dollar store or Wal-Mart!
